Adoption Application

Please fill out the application COMPLETELY. An incomplete application may cause a delay in processing. Your information is used exclusively by RMLR and will at no time be sold or available to third parties.

Please realize that we are a completely non-profit organization and our volunteers provide their own time, money, and gas to help out the organization. Please only apply for a dog if you are seriously interested. We don't want to wast our volunteers resources on applicants that aren't serious in adopting one of our labs and a family that is merely 'shopping'. Thank you for your application and your interest in rescuing a lab from the Rocky Mountain Lab Rescue.

Terms of Adoption (You must initial each box in order for your application to be processed)

The adoption fee for most Labrador Retrievers varies based on age, which helps to partially defray the veterinary and boarding expenses we incurred in rescuing and caring for the dog. Labrador Retrievers if old enough will have be altered, given all necessary inoculations including a Rabies shot, stool tested for parasites or given deworming medication, and given other medical treatment needed for problems RMLR is aware of and can treat before adoption. Any and all adoption fees are non-refundable.

If, for any reason, the adoption is not satisfactory for the Labrador Retriever or for the adoptive owner, RMLR needs to be notified as to where the dog would be rehomed. RMLR would prefer if you returned the dog. RMLR reserves the right to take back any dog if it is ever neglected, improperly cared for, or allowed to become a nuisance to others. Please note, if you return a dog, only half of the adoption fee is refundable and the other portion is used to care for other dogs of the rescue.

All dogs are microchipped before being adopted. They will be registered under RMLR’s contact information. If your Labrador is lost and is not wearing an appropriate ID tag, we will be contacted by animal control if the dog is picked up. We will return the dog to you for the cost of the retrieval fee from the shelter as we see fit for the dog.

If adopted, this Labrador Retriever must be licensed in accordance with existing laws in the adoptive owner’s town, county, and state, and the Rabies vaccination must be kept current for the dog’s life.

The adoption fee for puppies that have not been altered due to age covers all of their microchipping, inoculations, and deworming medication up to the point of adoption. RMLR will pay for the altering of your lab at our veterinarian at the appropriate age. An additional $100 will be collected as a deposit and guarantee that the puppy will be altered at the appropriate time. The $100 will be returned to you once altering has been completed. If you choose to have your Labrador altered at your vet rather than RMLR’s, RMLR will take your $100 deposit and pay it directly to your veterinarian and will require proof of altering.

Puppies more often, but all dogs can acquire Gairdia from standing water and Colorado soil. RMLR does not vaccinate for Gairdia as it is not always necessary. If your Labrador does contract Gairdia after the point of adoption, any medical costs will be yours.

The adoptive owner agrees to provide proper shelter high quality food as advised by RMLR, and proper medical care, and agrees that this dog will not run free without supervision, be chained out, be allowed around children under 14 years without adult supervision present (varies by dog), be allowed to ride loose in the back of a truck, or be used for guarding purposes. Failure to comply with these terms will require that the dog be surrendered immediately and returned immediately to Rocky Mountain Lab Rescue without refund.
The adoptive owner agrees to follow a regular inoculation program established by your veterinarian. Your dog will need an annual veterinary visit with Rabies boosters in accordance with provincial regulations.

Crating is an effective way of keeping your dog and home safe when you are not there. RMLR endorses the use of crates as a means of safety, training and security for the dog. Please talk to your RMLR contact during your phone interview about crating if you have any questions

Agreement

I attest all information provided above is true. I promise to care for my Lab to the best of my ability with the full intent of keeping the Lab for the duration of its life. I also agree to keep a buckle collar, ID tag and RMLR tag on the dog, provide quality dog food, adequate shelter, vet care and training as needed.  I understand that any inaccuracies shall result in my application being suspended.

I understand and agree that RMLR has the right to refuse my application if application or home situation is deemed by the RMLR as not appropriate for a rescued Labrador.  You also agree and give permission to RMLR to check with other rescue groups, humane societies, or my veterinarian and/or to share information about my application with said other groups or with my veterinarian.

By submitting this information, you agree to:

1. Not transporting this dog unconfined in an open vehicle.
2. Not leave the dog unattended in any vehicle, as temperatures can escalate to a fatal level within five minutes.
3. Regularly check the tightness of any collar and to remove choke chains or other training collars from the dog when the dog is unattended.
4. Seek assistance from knowledgeable sources, including but not limited to RMLR personnel, veterinarians, behaviorists, or obedience trainers when problems with the dog's behavior arise.
5. Comply with all local, state, and federal ordinances, laws, statutes, and regulations concerning animal ownership or guardianship, including but not limited to licensing requirements.
6. Provide suitable confinement for the dog. Under no circumstances is the dog to be routinely tethered as a means of confinement.
7. Provide for suitable care if the Adopter(s) will be absent from the home.

If adopting multiple Labs at one time, adoption fees may be reduced (as decided on a case by case basis by RMLR Board). All adoption fees are pooled to provide medical care for all of our dogs. Except in special cases, all of our Labs are spayed/neutered and vaccinated before going to their new homes.
